We are delighted to welcome you to our practice and appreciate the trust you have placed in us by scheduling an evaluation and consultation.  We will make every effort to honor that trust by providing the quality of care you require and deserve. We pride ourselves in the caring attitude of our experienced staff, offering the latest in diagnostic technology and pain treatment.

An accurate record of your past and current medical condition is vitally important for your evaluation. While our paperwork is comprehensive, it is invaluable since many seemingly unrelated conditions can contribute to head and neck pain.  Because it takes between 30 to 40 minutes to complete the paperwork, you will find that it is much more time-efficient, comfortable and convenient to do it at home at your leisure.  Please complete the information and bring it with you to your appointment so we can start your evaluation in a timely manner.

Our staff welcomes the opportunity to assist you in every way possible. If you have any questions that need to be answered before your appointment, please call us. Because many of our patients have medical conditions that are aggravated by scented products, our office is designated fragrance-free.  Please refrain from wearing any scented products to your appointments.

Your appointment will take approximately two hours.  Be sure to allow that much time in your schedule for us.  If you are unable to make your scheduled appointment, we would appreciate 24 hours notice so we may offer the appointment to another patient in pain.  We will then reschedule your appointment for a more workable time.

The patient registration and “Welcome to Our Office” are two separate attachments.  Please complete all the paperwork.  If for some reason the paperwork is difficult to read when you print it out, please contact our office.
